Name the pregnancy or reproductive history, menopause stage, family history, diagnoses, procedures, or treatment changes you want visible. Use the words you were given when you know them. “I am not sure” is also useful.

DAY 3 · FLOW
Gather the dated blood-pressure, cholesterol, and glucose information you already have. Record it without trying to interpret it yourself. A missing result can become a question.

DAY 4 · BEAT
If rhythm symptoms are part of your story, note when they happen, how long they last, what you are doing, and what you feel with them. Do not try to provoke or name the rhythm yourself.

DAY 5 · PUMP
List a heart test you remember, its approximate date, what you were told, and whether follow-up was recommended. Leave interpretation to qualified care.

Choose one familiar task you already do and describe whether it feels the same, easier, or harder than it used to. This is not an invitation to test or push yourself.
