Should women get a coronary calcium scan?

A coronary calcium scan earns its place when the result could change a real prevention decision—often whether to begin or intensify cholesterol-lowering treatment. Not every woman needs one.

For prevention, current U.S. guidance considers CAC for selected women 45 and older who do not have known cardiovascular disease or symptoms that need diagnostic evaluation. It is most useful when estimated 10-year risk falls in the intermediate range—or sometimes the borderline range—and the choice about cholesterol-lowering treatment is still unclear. Age alone is not a reason to have the scan. Family history, certain pregnancy complications, premature or early menopause, chronic inflammatory disease, and an elevated Lp(a) may change the context.

This is a selective prevention test. New chest pain or another urgent symptom needs prompt clinical evaluation; a self-scheduled calcium scan cannot answer that question.

What does a coronary calcium score show?

A CAC scan is a quick CT scan of the heart that usually does not require an IV or contrast dye. It looks for calcium inside plaque in the coronary arteries—the arteries that supply the heart muscle. The result is commonly reported as an Agatston score.

A higher score means that more calcified coronary plaque is visible. Across large studies, higher scores have gone with higher average rates of later heart and blood-vessel events. That is why the result can help refine a prevention conversation.

The distinction matters: a risk estimate is probability; a calcium score is anatomy. The scan shows one kind of plaque that is present now. It does not explain why it developed, show your lifetime cholesterol exposure, measure the heart's pumping strength, or prove that a specific narrowing is blocking blood flow.

What do different calcium-score ranges mean?

Here is what many women do not realize: a calcium score is not the percentage of an artery that is blocked. It measures the burden of calcified plaque the scan can see.

A score of 0 means the scan did not detect calcified coronary plaque. A score from 1 to 99 means some calcified plaque is present. A score of 100 or more usually carries more weight in a prevention discussion, and still higher scores identify a larger burden of calcified plaque.

Those numbers should not be read alone. Age, sex, blood pressure, cholesterol, diabetes, smoking, kidney health, family and pregnancy history, symptoms, current treatment, and the reason the scan was ordered all change the conversation. Reports may also include a standardized percentile comparing your score with a reference group based on age, sex, and race/ethnicity. The absolute score and the percentile answer related but different questions.

A high score does not automatically mean you need a stress test, an angiogram, or a stent. It is also not a prescription. Do not start aspirin, change a statin, or arrange more testing from an online chart without the clinician who knows why the scan was ordered and what decision it was meant to inform.

What does a zero calcium score not rule out?

Zero is often reassuring, especially when a scan was used to clarify prevention in someone without symptoms. It means no calcified plaque was detected. It does not mean the coronary arteries have been declared permanently clear.

A CAC scan does not detect noncalcified plaque, so a score of zero does not rule out all coronary atherosclerosis. That limitation can matter more in younger adults, including younger women, because plaque may exist before much calcium has formed. A zero score also cannot rule out coronary microvascular disease, coronary spasm, an intermittent rhythm problem, a valve problem, or every cause of chest discomfort or breathlessness.

A reassuring prevention test should never be used to dismiss a changing symptom. New or persistent symptoms need a clinical question of their own, and sometimes a different test.

When can CAC change a prevention decision?

For some adults at borderline or intermediate risk, a score of zero may support waiting on cholesterol-lowering treatment after a shared decision and repeating the scan in three to seven years. But zero does not carry the same reassurance for everyone. Important exceptions include familial or severe hypercholesterolemia, diabetes after age 40, current cigarette smoking, and a strong family history of an early heart attack or stroke. Any CAC above zero can strengthen the case for cholesterol-lowering treatment; a larger burden carries more weight.

The value comes from changing a decision, not simply producing another number. If treatment is already clearly indicated, if your risk is already very low, if you have known coronary disease, or if you have symptoms that need diagnostic evaluation, a calcium scan may add little or answer the wrong question.

The scan uses a low dose of radiation, and the amount varies by scanner and protocol. Pregnancy must be discussed before any test that uses radiation. Insurance coverage and out-of-pocket cost also vary. These are practical parts of deciding whether the information is worth obtaining.

One question to ask before a coronary calcium scan

Ask this before scheduling it: ‘Which decision are we trying to make, and what would we do differently if my score were zero, modest, or high?’

That question protects you from collecting anatomy without a plan. It also makes room for the rest of your story: older and newer cholesterol results, blood pressure, blood sugar, Lp(a), ApoB when relevant, family history, pregnancy history, menopause timing, symptoms, medicines, and what your body can do now.
