THE QUESTION
Why is my blood pressure suddenly higher at 45?
THE SIMPLE ANSWER
Blood pressure often changes in midlife. The menopause transition may be part of that change for some women, but it is not the only possible explanation.
Age, family history, a history of high blood pressure in pregnancy, sleep, weight, alcohol, salt, stress, pain, medicines, and kidney or thyroid problems can also matter. One reading cannot tell you which part of the story is responsible.
WHAT WE KNOW
A long-running study of women across the menopause transition found several different blood-pressure patterns. One group had changes that were consistent with menopause making a contribution. That does not mean every woman follows the same pattern.
The 2025 ACC/AHA blood-pressure guideline recommends confirming suspected high blood pressure with readings outside the office. Good technique matters: the right cuff size, a few quiet minutes first, back and arm supported, feet flat, and repeated readings rather than one rushed number.
WHAT WE DO NOT KNOW YET
A period change and a blood-pressure change happening at the same time do not prove that hormones caused the higher number. A home cuff can also mislead if it is the wrong size or used incorrectly.
The pattern across days matters more than one isolated reading. A clinician can help decide whether the change is lasting and whether another cause needs attention.
WHY THIS MATTERS TO YOUR HEART
Blood pressure is the force your arteries and heart work against. When it stays high over time, it can add strain and raise the chance of heart disease, stroke, kidney disease, and other problems.
This is not a reason to panic over one number. It is a reason to notice the direction early enough to act on the pattern.
ONE QUESTION TO ASK YOUR CLINICIAN
My blood pressure has been higher since my periods began changing. How should I confirm the pattern, and what else in my history could be contributing?
